1.Who we are
This website is operated by Eastern Virginia Advanced Care, LLC ("EVAC," "we," "us"), a CARF-accredited behavioral health provider and a NBJPN Holdings LLC company, located at 2600 South Loop West, Suite 240, Houston, TX 77054. This policy applies to information collected through this website and its forms. It does not replace our HIPAA Notice of Privacy Practices, which governs protected health information (PHI) created or received in connection with treatment.
2.Information we collect
- Information you give us in our forms: name, email address, phone number, mailing address, preferred contact times, and the content of your message (Contact, Book a Consultation, Referral, Donation inquiry, Careers application, and newsletter sign-up).
- Information about a referred individual that a referring professional, family member, or agency submits through a referral form.
- Job application details, such as work history, licenses, and availability.
- Basic technical information collected automatically, such as browser type, device type, pages visited, and IP address, used to keep the site secure and working.
- Login information for authorized EVAC staff using the administrative area.
3.Please do not send detailed medical information through general website forms
Our Contact and newsletter forms are not designed for sharing detailed clinical information. Please share only what is needed for us to reach you. Our clinical team will collect health information through secure intake and treatment processes. If you are in crisis, do not use this website — call 911 or call or text 988.
4.How we use information
- To respond to your questions and requests and to schedule consultations.
- To process referrals and client intake for our services.
- To review employment applications.
- To respond to donation inquiries.
- To send newsletters or service updates you request (you can unsubscribe at any time).
- To operate, secure, and improve the website, and to comply with legal obligations.
5.How we share information
We do not sell or rent your personal information, and we do not share it for targeted advertising. We share information only:
- With service providers who help us run the website, host data, send email, or process forms (for example, our hosting provider and JotForm for online intake and referral forms), under obligations to protect it. Where a provider handles PHI on our behalf, we require a Business Associate Agreement as HIPAA requires.
- When required by law, subpoena, or court order, or to protect the safety of a person.
- As permitted by our HIPAA Notice of Privacy Practices for information that becomes part of your health record.
- In connection with a merger, acquisition, or transfer of our operations, subject to this policy.
6.Third-party forms and links
Some forms on this site (such as Client Intake and Referral) are provided by JotForm and are subject to JotForm’s own privacy and security practices as well as ours. This site may link to other websites; we are not responsible for their content or privacy practices.
7.Cookies
We use only the cookies needed for the website to work, such as security cookies and sign-in cookies for authorized staff. We do not use advertising cookies. You can block cookies in your browser settings, though some features may not work.
8.Data security and retention
We use reasonable administrative, technical, and physical safeguards, including encrypted connections (HTTPS) and restricted staff access. No system is completely secure, so we cannot guarantee absolute security. We keep information only as long as needed for the purposes above or as required by law, including Texas medical-record retention rules for information that becomes part of a health record.
9.Children
This website is intended for adults. Services for minors are arranged through a parent, legal guardian, or authorized referral source. We do not knowingly collect personal information online directly from children under 13.
10.Your choices and rights
You may ask us to access, correct, or delete personal information you submitted through this website, or unsubscribe from emails, by contacting us at [email protected] or 346-618-0372. Rights regarding your health records are described in our HIPAA Notice of Privacy Practices. Texas residents have additional protections under the Texas Medical Records Privacy Act.
11.Changes to this policy
We may update this policy from time to time. The effective date at the top of this page shows when it last changed.
